At Praxis Solutions, we recognise that families and young people often require support during periods of transition, uncertainty, and change. Our Transitional and Family Services provide early intervention designed to strengthen family relationships, build capacity, promote stability, and support positive long-term outcomes for children, young people, and their support networks.

Guided by trauma-informed, neuroaffirming and strengths-based practice, we work alongside families, carers, communities and professionals to identify practical solutions that respond to each family's unique circumstances. Our focus is on building resilience, strengthening natural support networks, and creating pathways that help children and young people remain connected to family, culture, community, and opportunity.

Whether supporting a family to remain safely together, assisting a young person to transition toward independence, strengthening a kinship placement, or providing targeted outreach support, our goal remains the same: to provide the right support, at the right time, for as long as it is needed.
